All care staff share responsibility for maintaining skin integrity and wound care, within their level of training, with specialist tissue viability teams providing additional support when needed.
Wounds change over time. Regular assessment helps track healing, detect deterioration or infection early, and ensures treatment remains effective and appropriate.
Poor nutrition, infection, age, underlying conditions, smoking, certain medications, stress, and lack of social support can all delay wound healing and increase complication risks.
