Clinical risk assessment is a structured process used to identify potential risks to a patient or others, consider risk factors, and plan appropriate measures to manage and reduce those risks.
Static risk factors are fixed and cannot be changed, such as past events or history. Dynamic risk factors can change over time, such as a patient’s mental state, behaviour, circumstances, or environment.
Dynamic risk assessment involves continually assessing risks as a patient’s circumstances, behaviour, environment, or condition changes, allowing appropriate action to be taken when new risks arise.
The 3As model involves Anticipate, Assess, and Act. It helps healthcare professionals identify potential risks, evaluate the situation, and take appropriate action to manage or reduce those risks.
A risk assessment should be accurately documented, including identified risks, relevant factors, actions taken, and management plans. It should be reviewed regularly and whenever the patient’s circumstances or level of risk changes.
